The i-Clarity Clinical Records module consists of 4 record types: Pre-Screening, Clinical Records, CL Records and Non-Sight Test.

Note that patient information in the Clinical Records module is more sensitive than that stored in other parts of the system. It is strongly recommended that you use Maintenance > Users > Form Permissions to create a user group for registered staff. You can then assign your professional staff to that group and ensure that only they have access to the full clinical records forms (CL Clinical Records, Clinical Records and Non-Sight Test form).

 

Minus Cyl vs Plus Cyl

Throughout i-Clarity, the system defaults to either plus cyl or minus cyl according to the preference of the logged in user. This is set up in the User tab in Maintenance.

Pre-Screening

The Pre-Screening module is intended for practices where other members of staff, other than the Optometrist, start the examination process outside of the test room. It allows an Optical Assistant to gather initial information, allowing the Optometrist more time to concentrate on the examination and refraction.

If you are using Pre-Screening in your practice, you should complete the Pre-Screening form before the full eye examination, otherwise there is the possibility that data entered into the full record will be overwritten by information from the pre-screening.

To open Pre-Screening, click on the Exams button on the patient screen.

Then click on New Pre-Screen.

This will open the Pre-Screening form for the active patient.

The behaviour and use of each of the Pre-Screening fields is described below.

Previous Prescriptions - All previous prescriptions are shown in descending order in the Previous Prescriptions / Exams section. These can be scrolled through either using a mouse or the up and down arrows on the keyboard. The buttons at the side can be used to only show Eye Exams, CL Exams, Dispenses or Focimetered Spectacles.

Note - You can also automatically import details into this box from a connected, compatible focimeter by clicking the 'Equipment Import' button.

Objective - You can enter details of the Objective Rx in the appropriate boxes in the Objective section of the form.

Note - These values can also be entered automatically from a compatible, connected autorefractor, by clicking the 'Equipment Import' button.

Presenting VAs / Pachymetry - The PVA (Presenting Visual Acuity) and PNVA (Presenting Near Visual Acuity) and the Right and Left Pachymetry values can be entered here.

These may also be imported automatically from a connected, compatible instrument by clicking the 'Equipment Import' button.

Pressures - The Pressures on the Pre-Screening form is displayed as shown below.

If you click on the (+) icon, the following pop up will appear.

There is no need to enter the Date field, as i-Clarity will default to the current date and time (although you can change this if you wish). You can select the instrument from the Instrument drop-down. You can then type in the R and L average values and then click 'Select' to save the pressures.

Pre-screen Notes - This is a text field that can be completely blank so that you can type whatever you want into it. Alternatively, you can used pre-created templates that have appropriate questions and (optionally) 'tags' - defined by right and left chevrons like these: << >> - that allow you to select from predefined lists of answers.

If you use a template with tags, like in the example above, you can select from a pre-defined list of answers by clicking on the words inside the chevrons. This will bring up a list as shown below.

Patient Notes - This box can also be a free-type text box or can be populated by using pre-created templates that have appropriate questions and (optionally) 'tags' - defined by right and left chevrons like these: << >> - that allow you to select from predefined lists of answers. 

General Health - The General Health box is prepopulated with a '<>' tag as shown below. It is also possible to free type into this box.

When you click the tag, a list of predefined answers will pop up for you to select, as shown below.

Medication - The Medication box is prepopulated with a '<>' tag as shown below. It is also possible to free type into this box.

Family History - The Family History box is prepopulated with a '<>' tag as shown below. It is also possible to free type into this box.

Ocular History - The Ocular History box is prepopulated with a '<>' tag as shown below. It is also possible to free type into this box.

Patient's Occupation - This allows you to specify the patient's occupation from the pre-defined list of occupations. This updates the main patient record.

 

Form Buttons

These buttons allow you to change the size of the text. The size you choose will be retained when you save the record. Note that you can specify the default text size for each user, by selecting the User tab in Maintenance and entering the default size in the Font Size column. (The default is 10).

This button allows you to select the appropriate template for this examination, or a different one from the default if a default is defined for the current user. Note that you should choose your template BEFORE you start entering data, as selecting a new template will overwrite any data that you have already entered.

Once you have completed the clinical record, there may still be unused tags that you have not used because they were not necessary on this occasion. This button removes unused tags so that the final record is clearer.

Note that this will NOT remove tags that are listed as required for compliance purposes. Also, if you wish to reinstate a tag, then you can press Ctrl + Alt + T to display the full list of tags appropriate for the current field.

This button allows you to turn the spell check on or off.

The Imaging button allows you to access the i-Clarity Imaging Menu, which allows you to take, store and review imaging data from a variety of different systems including fundus cameras, sit lamps and OCTs.

The Go button allows you to leave the pre-screen record open and switch to the Exam Summary, close the pre-screen record and go to Sales & Payments, Dispense, Orders or Claims, or close the pre-screen record and go to the Exam Summary.

The Save Record buttons allows you to save the current pre-screen record and continue working on it.

The Close button allows you to quit the Pre-Screen Record.

 

Finalising the Pre-Screening

When you have finished completing the pre-screening record, you just save the record and press Close.

 

NEW ENTERPRISE FEATURES IN THE PRE-SCREEN RECORD

There are a few new features in Enterprise that have enhanced the pre-screen record and made navigating the form and the system much easier. These are - 

The Enterprise Menu at the top right of the screen.

You can use this to access the Diary, WalkIn Sales, the Refresh button, the Screen Lock button, as well as accessing other i-Clarity modules as shown below.

Patient Details, Communication, Documents and Utilities.

Clicking on the patient drop down on the top left of the screen brings up the menu shown below. This makes it easier to show and edit patient details, save, send and view documents and send communications to the patient.

Voice to Text.

The voice to text function allows you to dictate what you want to input into the text box within the clinical record, rather than having to type it in. This is available for all free type text boxes in the record by clicking the button shown below.

Translate to another language.

Clicking the Translate To Another Language button will bring up a list of languages that the text can be translated into. This is particularly useful for Welsh practices that have to use both Welsh and English.

Electronically Read The Text In English

Clicking the button below will read the text aloud in English. You can also right click the button to change it to a different language.
