The patient history button shows a grid of the changes that have been made to the patient record. The top line shows the information that was on the patient record before the last change was made, the line below that shows the information on the patient record before the change was made and so on.

You can access the patient history by clicking on the patients 'Further Details' tab and selecting 'Patient History'.
